A good disability case requires documentation. We offer a variety of forms to help you support your case.

Medical Source Statements

Arguably the most helpful piece of evidence in a case is a good word from your doctor. If your physician is willing, ask him or her to complete a questionnaire about your work-related limitations. You can download a generic form for him or her to complete here. Be advised, however, that this is a general purpose form and may not be relevant to the specifics of your case. Please contact our firm for a more detailed questionnaire that addresses your specific medical impairments.

Third Party Statements

It's also helpful to have a sworn statement by a neighbor, friend, or family member who is familiar with your condition. We recommend they write a statement on your behalf based on the example provided here. In our opinion, specific examples like those in the example above help to illustrate the extent of your limitations and disability.

Employer Statements

If you lost your job because of your medical impairments, that can serve as persuasive evidence that you are no longer able to work. And even if you're still working, it can be helpful to document any workplace accommodations provided by your employer. Financial Assistance

Vocational Rehabilitation

If you need help finding a job that can accommodate your disabilities, or for free workplace training, contact the Texas Workforce Commission's vocational rehabilitation program at https://www.twc.texas.gov/jobseekers/vocational-rehabilitation-services. Medical Assistance

One of the most common reasons a disability case is denied is due to a lack of medical care. If you need regular medical care, you should first see if an affordable health insurance plan is available through the ACA exchange by visiting www.healthcare.gov. The ACA provides a variety of plans on a sliding scale, and an affordable option may be available.

However, if you cannot afford medical insurance, there are other options. You can visit http://txclinics.dshs.texas.gov/chcl/ to search for indigent healthcare services based on your location. And here are some additional low- or no-cost options available in the North Texas region:
